1. ASSUMPTION OF RISK

INITIAL:  

I understand that participation in mixed martial arts, sparring, grappling, and open mat activities is inherently dangerous and involves known and unknown risks.

These risks include, but are not limited to:

• Serious bodily injury
• Broken bones, sprains, or dislocations
• Head injuries, concussions, unconsciousness
• Permanent disability or death

I knowingly and voluntarily assume all risks, including those arising from:

• My own actions
• Actions of other participants (members or non-members)
• Negligence of released parties
• Improper or inadequate supervision
• Equipment or facility conditions

2. NON-MEMBER / FREE SESSION ACKNOWLEDGEMENT

• Any claims arising out of participation

4. AGREEMENT NOT TO SUE

I agree that I will NOT sue or bring any legal claim against TEAM QUEST ATHLETIC CENTER LLC or any released parties for injuries or damages sustained during participation.

5. INDEMNIFICATION

I agree to indemnify, defend, and hold harmless TEAM QUEST ATHLETIC CENTER LLC from:

• Any claims brought by me or others
• Any damages caused by my actions
• Legal fees or costs arising from my participation

6. MEDICAL FITNESS & RESPONSIBILITY

• I have no condition preventing safe participation
• I assume full responsibility for my health

I understand that:

• The gym does NOT provide medical insurance
• I am responsible for all medical costs

7. RULES & CONDUCT AGREEMENT

• Maintain hygiene and safety standards
• Respect all participants and staff

I understand I may be removed immediately without warning for unsafe behavior.

8. NO RELIANCE ON SUPERVISION

I understand that during open mat, sparring, or free sessions, supervision may be limited or absent.

I agree that:

• I am solely responsible for my participation decisions
• I will stop if I feel unsafe
• I will not rely on staff to control other participants

9. PHOTO / VIDEO RELEASE

I grant permission for use of my image for promotional purposes without compensation.

10. SEVERABILITY

INITIAL:

If any part of this agreement is found invalid, the rest remains enforceable under California law.

11. FULL UNDERSTANDING
